Healthy Steps for Older Adults

CONCORD — Not-for-profit home health and hospice agency Granite VNA provided more than $6.3 million in community benefits during fiscal year 2024 (October 2023 – September 2024). Charitable programs and services included community health services, foot care clinics, immunization clinics, nutritionist services, palliative care and subsidized care throughout the 72 communities the agency serves across central New Hampshire.

“A critical part of our mission at Granite VNA is to enhance dignity and independence for people by delivering quality health care, regardless of a person’s ability to pay, and promoting wellness in homes and communities through every stage of life,” said Granite VNA President and CEO Beth Slepian. “We are proud of the work we’ve done this year to care for the communities of our region, providing services and programs to support and enhance wellness for all.”

Granite VNA’s key charitable investments included:

  • $2,369,060 in subsidized health services as a result of operating the Hospice House in Concord 24-hours-per-day, seven-days-per-week, year-round;
  • $1,060,000 in government-sponsored health care;
  • $245,009 in leadership development programs and trainings;
  • $183,940 in providing clinical opportunities for undergraduate clinician training;
  • $134,800 in charity care to individuals who are uninsured, underinsured or otherwise unable to pay out-of-pocket fees for service;
  • $126,438 in educational programs for community members of all ages to improve their quality of life and remain independent.

In addition, Granite VNA made 141,235 visits to 8,039 home care patients in their homes, facilities and retirement communities, as well as 45,288 visits to 1,251 patients on hospice in homes, assisted living communities, retirement communities, skilled nursing facilities, and at Granite VNA Hospice House. These visits totaled 1,575,203 miles of travel for the Granite VNA team. Further key figures include:

  • 1,305 community members participated in 147 group and online community wellness programs;
  • Granite VNA nurses made 908 “Baby’s First Homecoming” in-home visits to new mothers and their infants to help them safely transition home from Concord Hospital;
  • Granite VNA’s award-winning volunteers generously gave 8,911 hours of their time and talent to help the agency achieve its mission.
