Granite Pathways is Nashua's doorway at 12 Amherst St. It complements existing programs in place across the city. While working at her desk, Michelle Poulin, case manager with The Doorway at Granite Pathways, said the organization assists clients with treatment navigation, with any primary care doctors, any Medicated Assisted Treatment and setting people up with any necessary referrals. This doorway connects clients to the appropriate level of care and services. Additionally, Poulin said people are transitioned to an after-care case manager who can carry that caseload for up to a year.
