CONCORD — Granite VNA has introduced telehealth services to provide continuous support and monitoring for home care patients managing chronic conditions such as congestive heart failure, chronic obstructive pulmonary disease, chronic kidney disease and others.
Patients with chronic health conditions who are receiving care from Granite VNA’s home care team will now benefit from extended support from Ibis Health, a family-owned digital medical company serving patients on Medicare. Ibis provides daily vitals monitoring, medication reminders, and support via a tablet device. Patients also have access to Ibis Health’s specialized clinical care team who can provide health coaching and personalized goals.

Working in collaboration with patients’ Granite VNA home care teams and primary care providers, Ibis Health monitors patients 24/7 and alerts their care teams to any changes in their condition. Even after patients are discharged from Granite VNA care, Ibis Health continues to support them via telehealth to ensure uninterrupted care in periods of greatest vulnerability. And Ibis Health can quickly and easily reconnect patients to Granite VNA following discharge from home care services, which helps to reduce hospitalization.
Granite VNA’s telehealth services through Ibis Health are a Medicare Part B benefit. Ibis Health also accepts Medicare Supplemental plans and there is no upfront cost to join.
