FRANKLIN — Franklin VNA & Hospice is hosting a free Advance Directives Session at its 75 Chestnut St. office in Franklin on Wednesday, Oct. 10, from 10 a.m. to noon. A social worker will be available to answer questions, notarize advance directives, and provide information to help complete advance directive forms.

Advance directives outline the care wanted, or not wanted, in a situation where an individual cannot speak for him or herself. The advance directive also documents the chosen decision-maker.

Policies and procedures for emergency situations and healthcare needs vary, so it is important to leave clear instructions. Putting advance directives in place establishes a clear decision-maker if an individual is unable to make decisions on their own.
